Today’s lesson is that a qualified, experienced GP cannot detect if a patient has been sodomised.
Apparently, if the Hospital Pusrawi folk are to be believed, that requires a sodomy specialist… err, I mean a gut specialist. This from the Malaysiakini interview:
What led the doctor TRO (to rule out) sodomy? Why?
Because Saiful mentioned that he was sodomised. Sodomy check-up has to be done by a specialist not a medical officer. You need a gut specialist. We don’t have one in Pusrawi.
At the rate we are being enlightened on this err, delicate issue, Malaysians are going to be sodomy experts in no time. Maybe we could be a world leader in sodomy medical research – you know, sort of carve out a specialist niche in sodomy detection and patient therapy. After all, it appears to have become a national obsession.
Even our political vocabulary has been transformed. Thus, for example, instead of saying that the rakyat are being squeezed or suppressed, the popular and preferred usage now is the rakyat are being “sodomised”.
Anyway, this segment of the Malaysiakini interview brings back fond memories:
You have spoken to Dr Mohamed who has confirmed that he examined Saiful and confirmed that he has signed this medical report, why is the hospital still not convinced that it is the genuine medical report? Why is it taking so long to match the copy with the original copy?
We just realised this. When? Two days ago. It is just like VK Lingam said, “It looks like me, sounds like me but it is not me”.

On another front, Farish Noor has some questions for the wise Pas leaders involved in the Pas-Umno talks:
- What, in the respected opinion of the esteemed men of knowledge who lead Pas, is the correct hudud punishment for someone who kills a Mongolian model and then has her blown to bits by explosives (C4, for instance)?
- In the learned opinion of the respected men of knowledge of Pas, what should an Islamic Pas-Umno government do with weapons that were bought from Russia, a state that was known for its rather nasty habit of murdering Muslim Chechnyans?
- Would the wise men of Pas consider buying some more armaments from our beloved Russian allies in the future when they come to power as the bedfellows of Umno, such as, perhaps, used Kalashnikovs or other weapons of destruction that were perhaps also used to kill Muslims in Chechnya too?
- The Umno-led government has been praised by Washington as a key strategic ally in the ‘War on Terror’ and its leaders have demonstrated their eagerness to work with other developed Western powers in this war. Pas, on the other hand, declared the Taliban to be their brothers in 2002. Will the new Pas also join hands with Umno in the great Washington-led Crusade against Islamic terror when they come to power?
- Pas in the past condemned the Umno-led government for its alleged dealings with Israel. If Pas comes to power, will the learned wise elders of Pas find the necessary scriptural sources to help promote further dealings with Israel too?
- Between 2004 to 2008, the wise men of Pas declared that Islam Hadari was fundamentally unIslamic and when Pas came to power in several states in 2008 it even declared that it would no longer help promote the idea of Islam Hadari. Now that Pas may be on the threshold of its new honeymoon period with its beloved Umno, will Pas finally admit the error of its ways and accept that Islam Hadari is indeed the truest expression of normative Islam and do its utmost to promote Islam Hadari to its own misguided followers?
- Pas’ leaders have claimed that they wish to further the Islamic cause by getting closer to power. Will they now help to Islamise the Internal Security Act and other repressive laws to show that the ISA is, after all, compatible with Islam (especially Islam Hadari)?
- When Pas ousted its fourth President Asri Muda at the Pas assembly of 1982, its new Ulama leaders condemned Asri as a ethno-nationalist who sold the party to Umno and debased its Islamist ideology with ideas of racial communitarianism. Now will the same leaders of Pas please apologise to the departed Asri Muda and accept that Asri was right after all, and that the Ulamas’ ideology of Islamism was in fact an abberation that confused the minds of Muslims?

Just as there are forensic specialist MD’s who can diagnose rare diseases or if a rape is faked or not, so too I’m sure there are those who can pick up the truth about sodomy from detailed investigation and minute evidence which a GP may miss.
But, the point is Dr.Osman is a GP with over 15 years experience, and like many private practitioners, would have seen and treated just about every type of common and some uncommon afflictions as well, including piles, prostate enlargement etc which can cause swelling and bleeding and constipation. So, when in comes a patient who tells him point blank that he has been sodomised, is it likely that Dr. Osman would have given a perfunctory once over and then asked the patient to, pardon the pun, ‘bugger off’ to HKL?
The notes of his published MR clearly indicates that he had done quite a detailed examination b4 giving a verdict of TRO to sodomy. No plastic or other object was found in Saiful’s posterior end.
So, why the spin? Surely, there cannot be a suggestion of Dr.Osman being part of any conspiracy since Saiful DID NOT get the support he otherwise would have from him.
Looks like those with ambitions of being the next Minister of Health or in securing a billion ringgit direct-negotiation non-tendered govt or privatisation contract are making their play.
But, the plot curdles!! As posted earlier, this is sensational:

We do not need to expect any fairness from this country and their institutions. We are very sure at the end of the day Anwar will be found guilty.
A legitimate medical report is before us. The Police is calling that a sabotage. The hospital says it was not done by a specialist so it could not be concluded as final. In your years of experience as journalist, have you ever come across such joke anywhere in the world? Looks like this is how Malaysian government plans to create a knowledge base Malaysian society. MY FOOT.
Baskets. They are making us nuts and I wonder if we Malaysians were actually nut; because otherwise we would not be writing and keep writing but allowing them to exploit.

All human beings anus gets reversely penetrated(passing motion) within a few days of birth and this act continues almost on a daily basis for the rest of their lives.
Most adults(male/female/trans) would have been reversely penetrated sometime(and some many times) in their lives with big hard feaces when they were constipated.
When you are constipated, your feaces is usually hard and dry ……. how many of us had tears?Some of us may have bled a little but the next time we went to pass motion, no more bleeding any more ……… leave alone blood after few days. For pus to form , you need a tear and infection to set which rarely occurs.
For all those who still doubt that someone can get sodomized without suffering any tear, bleed and pus formation need to know that anuses are being routinely penetrated in hospitals on a daily basis with metal proctoscopes which are 8-10inch long and about 1 inch in diameter, the obturator is removed once the proctoscopes is in and once deeper inside the anus – no tears, no bleeds.
ABOUT THE OUTPATIENT EXAMINATION
1)It was done either in outpatient and probably in an ER(A&E) room
2)It was done by a medical officer and not a specialist
3)The history taken was very minimal as rightly pointed out:
when it occurred ie time, date, place
how many times has it occurred
who did it
consensual forced or neither ie not agreeing but not resisting
any constipation recently or in the past
size, consistency , shape etc.
Physical examination PR means Per Rectal examination whereby the basic examination would be;
spreading the cheeks of the buttocks in a semi prone position to look at the anus under good lighting
putting a gloved forefinger through the anus to feel tone, swellings, irregular surfaces, prostate
proctoscope examination whereby a speculum is inserted through the anus to look directly at the anal canal and also the anus while the proctosocpe is being withdraw out of the anal canal after completing the examination. This is the appropriate exam to look for tears.
Looking at the OPD exam, it looks like the doctor just performed only the visual exam because:
all is reported as “seen/not seen”
no mention made of findings of finger rectal exam ie stained with mucus/blood/feaces etc.
no proctoscopy exam findings mentioned
It is surprising how casual the examination was in view of the fact that the patient complained of assault to his anus – doctor’s job is to do a thorough exam irrelevant whether he/she believes the patient complaint(s)
Finally, the examination was not done by a specialist who in this case would have to be a colorectal surgeon and a forensic specialist. Even if the specialist finds no tears(old or new), blood or pus, it does not mean a person was not sodomized.
A disservice has been done to both the patient and also the alleged assaulter by the very poor history taking as well as physical examination of the patient. It is not rare for doctors to take such poor history and do such poor examination in patients with running nose or cough of few days but to do so in a potentially medico-legal case is (allegedly) grossly irresponsible.
The doctor’s only service to this patient was that he felt that patient may have been sodomised [TRO Assault(Sodomise)]and thus needs to be properly examined by someone more qualified and experienced in such areas which rightly should have been a specialist from the same hospital.It is a well known fact that private hospitals have briefed their A&E MOs to refer any potentially medico-legal case which do not have life threatening problems to government hospitals to avoid any hassles later on.
As a medical doctor, i feel the slipshod OPD examination should be properly addressed and explained by the Malaysian Medical Association so that no party(accuser or the accused) can twist the facts any further.
Truth will ultimately prevail.

Why are the police taking so long to conclude their investigations on the alleged sodomy by anwar?
This is a straight forward case. You have one man accusing the other of sodomising? They need months to solve a straight forward case?
How can people trust the findings of the police now? Everything seemed to be doctored, the credibility of the entire case is questionable.
They say that the doctor is not a specialist, but looking at the medical slip, the doctor definately took a look at the guys anus.
What expert analysis do you need?
Do they think that people are so stupid…?
Did that guy even look traumatised or sodomised to begin with?
The country has better things to do than to continue with this case.We are now an international joke.

I am not a medical man and I must admit the subject Sodomy specialist is completely foreign to me. So to satisfy my curiosity, I made a few enquiries with my Australian doctor friends on this ‘noble’ profession. They gave me a cheeky laugh and asked me, “Are you joking?” In reply jokingly, maybe, you should ask the gays!

The government is (again) trying to fix DSAI, but are finding it hard, as this time not enough preparation is made and internet “penetration” is far more spread. This is exaclt the question I am asking myself. Where can we find specialist in sodomy in the whole world?
Let us say a boy is sodomised in far off interior of Sarawak. Where is he going to be examined? Would Syed Hamid or err that Pusparwi fella fly him to see a specialist in sodomy.
Anyway I like to pose this question to Pusparwi hospital’s general manager Wan Mahmood Wan Yaacob, where can we find sodomy specialist? What is called? Sodomithist? I very much like to know, Mr Pandai Wan Mahmood? I have looked up dictionaries and still could not find one. Please show us!!!!

If the other medical report states that Saiful was indeed sodomized, then the question is what happened in the 4 hrs between the first medical examination at Pusrawi & the second one at HKL.
